Some members are eligible for extra care. LTSS helps older adults and people with disabilities live at home for as long as they can. The program provides and coordinates primary, acute, behavioral and long-term care. You can get these services in your home or assisted living center. You don’t need to live in a home, community, nursing home or other facility. Services may include:
